• A few years ago, UIL decided that students in marching band needed to have a physical just like kids in athletics. 

    Here is the UIL physical paperwork for the parent and the doctor to fill out.  The parent needs to fill out the "Medical History" side, and the doctor needs to fill out the "Physical Examination" side.  Practically every place that gives school physicals will have this form in their office.

    IMPORTANT NOTICE:  Physicals are normally good for 2 years. However, if you answer "yes" to any of the first 6 questions on the parent part, you will be required to get a new physical.

    Failing a physical exam, means you can still be in band, but you can not march.

    Not getting a physical exam, or not filling out the "Medical History" form means you will be removed from band.
